Position Summary:

We are seeking a mission-driven, compassionate High Acuity Social Worker to join our team supporting people living with HIV (PLWH). You’ll work in a collaborative, multidisciplinary setting to provide individual therapy, crisis support, and guidance to Community Health Workers (CHWs) serving clients with complex social and behavioral needs.

This position centers harm reduction, trauma-informed care, and low-barrier services, with a focus on equity for LGBTQIA+, BIPOC, immigrant, and homeless individuals.

Key Responsibilities:

  • Conduct assessments and intake interviews
  • Provide short-term, low-threshold therapy (caseload ~20 clients)
  • Support CHWs in developing and implementing service plans
  • Supervise CHWs and provide regular case coaching
  • Provide crisis intervention and emotional support
  • Maintain timely and accurate documentation
  • Contribute to the development of support groups and wellness initiatives

Qualifications:

Required:

  • MSW + LCSW (MA)
  • 2+ years experience in social work or behavioral health
  • Strong commitment to harm reduction and trauma-informed models
  • Experience working with LGBTQIA+ individuals and people of color

Preferred:

  • LICSW
  • Supervisory experience
  • Familiarity with Ryan White services and public health documentation
