Job Description

PROGRAM/DEPARTMENT DESCRIPTION

As a member of the Division of Education and Community Engagement, the Coordinator III, in conjunction with the Senior Training Program Manager will develop and implement educational initiatives, multi-session training institutes as well as shorter workshops with BPHC colleagues and partners on topics related to HIV, infectious disease prevention, health equity and racial justice. The Case Management Training Program is a Ryan White Part A funded program created to provide training to providers funded to deliver Medical Case

Management (MCM) and non-Medical Case Management services to People Living with HIV (PLWH) within the Ryan White Boston Eligible Metropolitan Area. The PCIII will partner organizations to provide training to case managers, to ensure PLWH can reach and maintain viral suppression by accessing the full continuum of HIV services.

DUTIES

In addition to all duties in Program Coordinator II, Program Coordinator III will perform these duties:

  • Takes a leadership role in ensuring that case management training requests are completed and adhere to HIV case management and Public Health Services (PHS) guidelines to improve case management services for people living with HIV.
  • Implement strategies for improvement to quality medical care including case management services including health-related supportive services to achieve viral suppression. This includes training providing training on needs assessment, motivational interviewing, individualized services plans, and appropriate access to behavioral health services, among other case management-related training.
  • Take on a leadership role to supervise the prompt fulfillment of case management training requests, ensuring compliance with HIV case management and Public Health Services (PHS) guidelines. This initiative is geared towards improving case management services for individuals living with HIV.
  • Implement strategies to improve the quality of medical care, including case management and supportive services, with the aim of viral suppression. This includes conducting training on needs assessment, individualized service plans, developing racial justice and equitable practice training for individuals living with HIV (PLWH/A), ensuring proper access to behavioral health services, and other relevant case management training.
  • Ensure that infectious disease case management training initiatives are organized, designed, and overseen, as well as monitor and evaluate infectious disease case management processes.
  • Assist in creating and modifying training materials and curricula to incorporate the most recent advancements in HIV disease management. Ensure that the training content complies with relevant regulations and guidelines.
  • Cooperate with both internal and external stakeholder groups, such as the Ryan White Services Division, healthcare providers, community organizations, and public health agencies.
  • Assist in the facilitation of training sessions and workshops, and report on program evaluation and activities. Analyze the effectiveness of training through assessments and feedback mechanisms.
  • Collaborate to steer and shape the programming initiatives for the Infectious Disease Case Management Advisory Committee and help co-facilitate program activities, evaluations, and the design of the advisory committee.
  • Working to help BPHC become an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.

MINIMUM QUALIFICATIONS

  • Master’s Degree in Public Health, Health Policy Administration, or a related discipline, such as sociology, organizational psychology, public health nursing, or health communications required; OR BA/BS plus 5 years of equivalent experience in public health program coordination, management and/or community organizing.
  • Licenses, certifications or program specific experience may be required by grant or program needs.
  • Experience designing and implementing public health or health related programs and initiatives.
  • Knowledge and experience in coalition building, community organizing and with health education and health promotion strategies.
  • Demonstrated experience coordinating and implementing complex projects in HIV.
  • Ability to write clear, concise reports, and to follow and to give written and oral instructions in a precise, understandable manner.
  • Demonstrated experience or related requirements as established by the program may be needed.
  • Experience planning community meetings and events.
  • Excellent organizational and oral and written communication skills.
  • Ability and experience working with diverse populations.
  • Excellent supervisory and team building skills including planning and assigning work; evaluating and motivating staff; determining the need for disciplinary action; and either recommending or initiating disciplinary action.
  • Advanced knowledge & experience with MSOffice and Knowledge of data analysis and epidemiology.
  • Advanced experience with and knowledge of local community agencies.
  • Promote health equity, inclusion, and diversity within the BPHC, department and community.
  • Knowledge of or willingness to learn about the role of public health in addressing racism, the social determinants of health, and inequities in health outcomes as well as strategies to advance racial justice and health equity.
